1. Register Through Our Secure Patient Portal
Once you have received your patient portal login details, please sign in to your account. Go to the “Notifications” section on the main dashboard, where you can complete and submit essential information, including:

- Emergency contact
- Medical history
- New patient forms
2. Completing New Patient Forms
If you are unable to complete your registration electronically, please visit our Patient Forms page, where you can print and complete the necessary forms to bring with you.
If you cannot access the online forms or bring completed forms, please arrive 15 minutes early to complete the paperwork before your scheduled appointment time.
Language Assistance
To ensure effective communication, we offer on-demand interpreter service platform, providing access to language support as needed during your visit.

Your initial physical therapy appointment at StarPro will include:
